SUSPENSION, WITHDRAWL OR REINSTATEMENT OF CERTIFICATE
PROCEDURE:
Suspension of Certification
A certificate may be suspended in cases such as:
- Non-compliance with certification requirements
- Failure to allow surveillance or recertification audits
- Misuse of certification mark or logo
- Voluntary suspension requested by the client
Process:
- AVSMS Identifies the reason for potential suspension.
- Notify client in writing with evidence and required actions.
- Allow a defined response period (e.g., 15 working days).
- Decision made by AVSMS Certification Decision Committee.
- Inform client of suspension decision and update the public directory.
- Monitor client for resolution within the allowed suspension period (max 6 months unless justified).
Withdrawal of Certification
Occurs if:
- Client fails to resolve suspension cause within specified timeframe
- Company ceases operations
- Certification is misused or falsified
- Voluntary withdrawal by client
Process:
- AVSMS Reviews the unresolved issue or withdrawal request.
- Document findings and recommendations.
- AVSMS Certification Decision Committee finalizes withdrawal.
- AVSMS Notifies client in writing.
- Update public records and request return or destruction of certificate.
Reinstatement of Certification
Criteria:
- Suspension period not exceeded.
- All corrective actions are verified and accepted.
- A follow-up audit or desk review confirms compliance.
Process:
- AVSMS Receives corrective action evidence or re-audit request from client.
- AVSMS Conducts review or audit as needed.
- Technical Reviewer confirms compliance.
- AVSMS Certification Decision Committee approves reinstatement.
- AVSMS Notifies client and update certificate status.
